Before the First Visit

New Patient Forms

Please download and fill out both the Intro Packet and Patient Health Questionnaire, detailing your past medical history, present chief health concerns, current supplements and/or medications. Additionally, you will want to contact your healthcare provider(s) to obtain your recent medical records including laboratory and diagnostic test reports that will allow the doctor to gain a better understanding of your current health picture.

First Visit

During the visit, the doctor will address your main concerns from the Patient Health Questionnaire. The doctor will complete a thorough health history with great detail, complete any necessary physical exams, and order any pertinent labs. Following your initial visit, you will complete a one-week diet diary to be brought to your next visit and the doctor will create a health care plan catered to your needs, taking into consideration your entire health care picture, including strengths and weaknesses. Basic treatment guidelines may be presented.
(First Visit — Up to 90 minutes)

Second Visit

The second visit takes place approximately 7-10 days after the initial visit. At this visit, the doctor will give a full assessment of your current health care picture, educate you on your current health picture, implement a plan of action, and discuss your expectations.
(Second Visit — Up to 45 minutes)

Return Visits

Subsequent office visits are based on your progress and are scheduled in 4-6 week intervals. Return visits allows the doctor to monitor your existing health picture and implement any necessary and/or beneficial changes.
(Return Visits — 15-60 minutes)

Phone Appointments

FREE 15-minute consultations are given for first time patients. No medical advice or diagnosis will be given during the consult. General phone appointments are currently only available for established patients. Please call the office for more details.

Cancellation Policy

Your time with the doctor has been reserved for you exclusively. If you need to change or cancel your appointment please call or email us as soon as you can. Please note that we have a 48-hour cancellation policy. We will attempt to fill cancelled appointments, if successful there will be no charge.

Parking

Street parking is residential 2-hour parking zone and metered parking. Paid parking is available at 1635 Divisadero St. The garage entrance is on Sutter Street. The garage opens at 6 a.m. and closes at 9 p.m., Monday to Friday. The rate is $5.25 an hour, with a maximum fee of $17 for up to 24 hours. Metered street parking rarely is available.

Public Transportation

MUNI’s 1 California, 2 Clement, 4 Sutter, 38 Geary (Regular and Limited lines) travels east and west. $2.00 Fare.
